To print out your own forms for YMCA Before/After School Child Care, just click on one of the links listed below. You must have Adobe Acrobat to print the forms. YMCA Before/After School program registration is on a first come/first served basis. Printing of a form does not guarantee a spot in the YMCA Before/After School Program.

To register for the YMCA Before/After School Programs, the forms needed are: Registration Packet, Health Assessment Form, and the Medical Emergency Consent Form.  The YMCA Child Care program accepts DHS child care assistance payment.  Financial assistance is also available for those who do not qualify for DHS assistance.  Please contact the Child Care Services Director at 366-6421 ext 102 to get more information.
